Insurance Benefits

Multicultural Affairs – Insurance/Benefits

  • $25 co-pay to provider on list (link below)
  • $50 co-pay to other providers
  • Ambulance 鈥 may pay up to $750.
  • ER deductible: $100
  • Prescription medication: 50% of actual charge. Must pay for prescription up front and submit claim to get reimbursement.

Claim information

  1. Complete this .
  2. Email the completed form along with any itemized receipts, bills from your doctor to service@hccmis.com.
  3. You can follow-up on your claim by calling 800-605-2282 or by logging into MESA.

MESA allows you to view the status of your claim right online by visiting . If you have not registered before, please follow the instructions below:

  1. Click on the “Sign Up Now” link to Register
  2. Select “Member” for the type of user
  3. Enter your email address, then click “Continue”
  4. Enter Certificate Number (this is the ID number on your insurance card) followed by two zeroes (00), First Name, Last Name, and Date of Birth (mm/dd/yyyy), then click “Continue”
  5. Select the appropriate matching record, then click “Continue”
  6. Enter your requested Username, Password & Password Confirmation, Security Question, Security Answer, then click “Continue”
  7. Complete the Electronic Authorization Signature and click “Continue”
  8. Click “Go To Login Page”
